Emergency Medical Information
In the event of a medical emergency, DPS will provide your emergency form to doctors or medical personnel. This form includes your insurance information, health issues, allergies, and medications. This form is designed to assist you and medical personnel in an emergency but it is not required. This form can be updated at any time through BANNERWEB .
